Policies
At Fargo center for dermatology, we strive to provide the best care and experiences for our patients and care team. In order to achieve that, we have established policies to ensure that our patients are able to obtain the best care and experience we can provide.
Cancellation/No-Show Policy and Fee
(Effective January 1, 2024) At Fargo Center for Dermatology, we require at least 24 hours’ notice if you need to cancel or modify your appointment. If an appointment is cancelled less than 24 hours in advance or patients fail to show up for a scheduled appointment, Fargo Center for Dermatology may charge a cancellation fee of (a) $50.00, for all medical appointments; (b) $100 for all surgery appointments, and (c) the lesser of $250.00 or 50% of the aesthetic service cost, for all aesthetic service appointments. This cancellation fee must be paid within 30 days of the missed appointment. Additionally, Fargo Center for Dermatology may refuse to provide additional services until any outstanding cancellation fee(s) have been paid. Coupon/Certificate
On occasion, Fargo Center for Dermatology offers promotional opportunities in the form of coupons or certificates. The following guidelines are standard to all coupons or certificates that are presented to be redeemed.
- Coupon/certificate must be present at the time of service. Service performed without the presence of coupon/certificate will be charged at full cost.
- Fargo Center for Dermatology is not responsible for replacement for lost or stolen coupons/certificates.
- Coupons/certificates hold no cash value and cannot be redeemed for the cash amount towards another service or product.
- Coupons/certificates cannot be purchased and redeemed on the same day.
- Coupons/certificates are not applicable with board-certified dermatologist Dr. Rachel Ness, FAAD.
- Expiration Date presented coupon/certificate is final.
Treatment Packages/Pre-Payment
Fargo Center for Dermatology offers select services the opportunity to be discounted through pre-paid offers or in packages. Occasionally these opportunities are a part of an event or offered to receive special pricing. Services that are purchased in packages, pre-paid, or purchased during a promotional event are subject to the following guidelines:
- Service must be performed within 6 months of pre-payment or package purchase.
- If services or treatments have not been utilized at the time of expiration, previously applied discounts will be forfeited, and the remaining balance will remain on the patient account for future services.
- Packages/pre-paid services are nonrefundable and may not be transferred to other accounts.
Authorization to Charge Credit Card on File
(Effective January 1, 2024) As a convenience to you, Fargo Center for Dermatology may offer to store your credit card information securely within the electronic medical record. If you elect to have Fargo Center for Dermatology store your credit card information, you authorize Fargo Center for Dermatology to charge this credit card for any amounts owed by you to Fargo Center for Dermatology, including co-payments, service charges, and cancellation fees. You may change or remove the credit card information stored by Fargo Center for Dermatology at any time by contacting Fargo Center for Dermatology.
Deposits
Select services require a deposit to be paid at the time of scheduling which is then applied as a payment towards the scheduled service once the service has been provided. Deposits range from $50 to $500, depending on the service, and are required to hold the appointment. Deposits are non-refundable unless the cancellation policy timeframe is allowed.
Financial Policy
It is the patient’s responsibility to understand and comply with any requirements of your individual insurance plan(s). Please know that you will be responsible of any payment due, including co-payments, co-insurance amounts, and deductibles for all visits and treatments provided. As a courtesy to our patients, we bill participating insurance plans on behalf of the patient if a valid insurance card is provided. Without a valid insurance card presented prior to the visit, a $200 deposit is required to be seen. If valid insurance information is provided within that same business day, the $200 deposit will be fully refunded. If you elect to self-pay, payment is due at the time services are rendered. When opting to self-pay, it is important to recognize that we are unable to bill any insurance coverage or HSA for historical visits after the fact. For parents or legal guardians consenting to care and treatment for minor children and/or dependent adults, the parent or legal guardian assumes all financial responsibility for services rendered. For Medicare and Medicaid participants with additional private coverage, allowed charges must first be billed through a private insurance plan with Medicare coverage secondary. You understand that Fargo Center for Dermatology is not a provider for Workman’s Compensation, and you will be held liable for the total amount of charges for services provided. If you have private health insurance, there are protections in place that provide visibility to your planned medical charges and fees when requested. If you’re uninsured or you decide not to use your health insurance healthcare services, under these protections, you can often get a good faith estimate of the cost of your care upfront, before your visit. Patients who are uninsured or who choose not to utilize insurance coverage for medical services have the right to request a good faith estimate for the total expected costs prior to receiving care. For cash-pay patients, a breakdown of costs for any tests or procedures beyond the overall medical visit charge is shared at the time of the visit. You can request a paper copy of this estimate from our office at any time. We will also keep a copy on file in your patient chart for future reference if need be. If the bill you receive is at least $400 more than your good faith estimate, you can dispute the charges. You have 120 days from the date on your bill to do so.
